Study Finds No Level of Alcohol Consumption Improves Overall Health

New research published in the Journal of Studies on Alcohol and Drugs found no overall health benefit from any level of alcohol consumption and linked even low intake to higher long-term risks of cancer, cardiovascular disease, and premature death.

The study, called the Alcohol Intake and Health Study, was led by researchers in the United States and Canada, including Kevin Shield, an associate professor at the University of Toronto and senior scientist at the World Health Organization/Pan American Health Organization Collaborating Centre in Addiction and Mental Health. The work was initially commissioned by the U.S. federal government to help inform the next edition of the U.S. Dietary Guidelines.

According to the researchers, people who consumed an average of 14 drinks a week faced an alcohol-attributable mortality risk of 1 in 25 over their lifetime. Drinking up to 7 drinks a week was associated with minimally elevated health risks for most conditions studied. The authors said risk rose as consumption increased.

“Even low levels of alcohol use come with health risks,” Dr. Shield said in comments released with the study. “And that risk continues to increase the more someone drinks.”

The researchers set out to estimate how lifetime drinking habits affect Americans’ chances of illness, disability, and death tied to alcohol. To do that, medical experts reviewed more than 7,200 scientific articles on alcohol-related diseases and injuries. The team then applied those findings to large national health data sets and used statistical modeling to estimate how different levels of drinking may shape long-term health outcomes.

The analysis covered chronic and acute conditions known to be causally related to alcohol, including cancers such as esophageal, oral, and breast cancer, as well as cardiovascular disease, liver disease, and injury. While low levels of drinking may be linked to lower risks of ischemic heart disease and stroke in some cases, the researchers said those possible benefits were outweighed when all alcohol-related harms were considered together.

“We did not observe a significant protective effect of alcohol on health at any level of consumption,” Dr. Shield said. “At low levels, alcohol may be associated with a reduced risk of ischemic heart disease and stroke. But when you look across the full range of health outcomes, including cancer and other chronic diseases, those potential benefits are outweighed by the risks, even at seven drinks per week.”

The findings challenge a long-standing public belief that moderate drinking may protect health overall. They also arrive as federal guidance remains broad. Current U.S. Dietary Guidelines advise Americans to limit alcoholic beverages but do not define a specific amount considered safe. Earlier versions recommended no more than two drinks a day for men and one for women.

Timothy Naimi, director of the Canadian Institute for Substance Use Research at the University of Victoria and an adjunct professor at Boston University, said the study was intended to provide a clearer quantitative framework than current federal advice offers.

“While the new U.S. Dietary Guidelines contain a useful ‘less-is-best’ message, they provide no quantitative framework,” Dr. Naimi said. “Our study was designed to do just that across the drinking spectrum.”

He added that “two drinks per day, which might be considered ‘moderate’ from a social standpoint, is associated with a substantially elevated risk of a premature death caused by alcohol.”

The authors said their estimates reflect population-level risk rather than a prediction for any one person. Individual outcomes can vary based on genetics, lifestyle, drinking patterns, and other factors. They also noted that research on alcohol continues to evolve and that newer evidence is linking drinking to additional conditions, including pancreatic cancer.

Even so, the study offers a benchmark likely to influence public health discussions because it suggests increased risk above one drink per day for both men and women. That could add pressure on policymakers and health agencies as they weigh whether future dietary guidance should include clearer thresholds or stronger warning language.

The findings may also matter beyond medicine. For wine, beer, and spirits producers, distributors, retailers, and hospitality businesses, stricter public messaging around low-level drinking could affect how products are marketed and discussed with consumers. It may also revive debate over health warning labels and moderation claims at a time when many beverage companies are already expanding low- and no-alcohol offerings in response to changing consumer habits.

The study’s authors said clearer information can help people make more informed decisions about drinking. Their central conclusion was direct: lower consumption means lower risk, and no amount of alcohol showed an overall protective effect on health in their analysis.
